Barrie and Community Familly Health Team - LINKS Navigation Clinic

Our LINKS Navigation Clinic assists patients with multiple, complex conditions and coordinates care between the hospital, the family doctor, the long-term care home and community organizations which results in these patients receiving better, more coordinated care. The clinic also helps navigate patients between the health care system and the social services system to reduce the burden of stress on patients as well as the burden of cost on our health care system.
