Bridges Community Health Centre

​Bridges Community Health Centre serves the communities of Fort Erie, Port Colborne and Wainfleet with a focus on the needs of seniors, children and youth, newcomers to Canada, and families or individuals with low incomes. Bridges Community Health Centre is committed to providing a full range of client-centred primary care services, particularly to those with limited access to health services. Bridges CHC Health Promotion and Community Development team consists of two Health Promoters and two Community Health Workers. Together with community partners they work to build healthy public policy, create supportive environments, develop personal skills and improve access to health services. They advocate for equity-based health services and social supports to allow people to have more control over the determinants of health and ultimately improve their health. Healthy eating and nutrition programs and counselling are led by Bridges CHC's Registered Dietitian. Programs are designed to support
